Current Patients: We use the CHARM Patient Health Portal for all communication.
Use the links below to download the Charm mPHR app from the Apple or Google Store for mobile use, or you can access from your browser.
For Billing Questions: Please message our practice administrator, Kristine Deytiquez, in the Charm Portal.
To join an appointment: All appointments are conducted via video, using Zoom. Your provider has link that never changes. Please check your appointment reminder email to find the link for your provider, or look in the “Announcements” section of the Charm PHR Portal.
For Medication Refills: click here.
To Reschedule: Go to the Charm App and click “Book Now” to book a new appointment time (or the Red Plus button on the appointments page of the browser portal). After selecting a new time, select your old appointment time and hit “Cancel your appointment”. Please do so with at least 24-hours notice to avoid a no-show or late cancel fee.
